Services received by you from Life Skills Counseling Center may be reimbursable by your health insurance provider. However, there are several important things for you to consider. 

Life Skills Counseling Center is a cash-based mental health practice. What that means is simply that Payment in Full is due at the time of service or prior to service for coaching services that are purchased through this website. We do not "take insurance" or file claims with your insurance company for you.  However, that does not mean that your insurance company will not reimburse you for professional counseling services with Life Skills Counseling Center. It simply means that you, as the patient, are responsible for submitting all forms that your insurance company requirs.  Although Dr. Gardner is Licensed by the State of Indiana as a Mental Health Counselor, he is not an approved provider for any health insurance company.  That simply means that he has chosen not to participate in managed care in order to offer the highest protection and confidentiality for his clients.

Life Skills Counseling Center is a managed-care free counseling practice. This means that his commitment is to providing the best treatment for you, while maintaining complete confidentiality of your personal information. In this arrangement, Dr. Gardner is free to work with you to make the best treatment decisions for your situation and avoid any third-party conflict of interest.

Once you file a claim for with your health insurance provider that claim is permanently in the system and forever part of your health record. As a licensed mental health counselor, I am required to provide a diagnosis on all insurance forms. If you choose to not file a claim, your case notes will simply remain under lock and key with Life Skills Counseling Center. Please note that there are certain legal situations where Life Skills Counseling Center would be required by law to release your case notes to the proper authorities. Please read the Informed Consent form in the Required Forms sections to understand what those are.

Please also note that certain services provided by Life Skills Counseling Center may not be covered by your health insurance provider. For example, Online Services (counseling by email) is currently not covered by insurance. However, due to its proven effectiveness and the growing use of Internet services, we are confident that this form of therapy will be reimbursable in the near in future. We encourage you to please contact your insurance company and tell them that you would like this form of treatment covered. Thank you.

Note that since we are licensed as mental health providers, our services are often covered covered under company "Cafeteria Plans."

It is up to you, the client, to determine whether or not our services are covered under any part of your health care plan.
